Influenza (flu) is a contagious respiratory illness caused by influenza viruses and can cause mild to severe illness. The best way to prevent the flu is by getting vaccinated each year. As we begin to enter flu season amidst the ongoing COVID-19 pandemic, it is important for everyone to get their flu shot to help protect yourself, your families, and Niner Nation.
This year, UNC Charlotte required all employees and students to attest that they have received a flu shot by Monday, Nov 16.
New Students and Employees in Spring 2021
Update: You will receive this form directly in your inbox by 5 p.m., Thursday, Jan. 21, and the deadline will be in early February.
There is no separate exemption form for the flu shot. You will complete the attestation form stating you did not receive the shot and indicating the accepted reason why.

Proof of vaccination
You will be asked on the flu attestation to provide the date and location of your shot. We do not require additional documentation of your vaccination.
Documentation of on-campus flu shot
If you received your flu shot on campus and need proof of vaccination for your off-campus employer, you may request documentation in one of the following ways:
- If you received your shot at the Student Health Center, contact them for your records.
- If you received your shot through an on-campus clinic, it was administered in partnership with Publix. You may request your vaccination record by visiting any local Publix pharmacy.
How you can get your flu shot
You may also contact the Student Health Center at 704-687-7400 to schedule an appointment for a shot. This includes faculty and staff.
